Frozen Semen Shipment Instructions

How to arrange a shipment

1. STEP 1

   Authorize the release

   The semen owner completes and signs a Frozen Semen Authorization Form, authorizing Canine Cryobank to release the stored semen, and faxes it to us.

2. STEP 2

   Gather the required information

   Have these ready: a FedEx account number for the shipment (call 1-800-GO-FEDEX or visit the FedEx website to set one up), the shipment destination and delivery information, and the name, address, telephone number, and email address of both the semen owner and the bitch owner or new semen owner.

3. STEP 3

   Complete the Frozen Semen Order Form

   The bitch owner or new semen owner fills in the order form with all of the information above. Processing cannot begin until everything requested has been supplied.

4. STEP 4

   Print, add payment, and sign

   Print the completed order form, fill in the payment information, and sign it.

5. STEP 5

   Fax everything to us

   Fax the signed order form and the authorization form to Canine Cryobank, Inc. at 760-591-9939. Shipment processing begins once both documents are received.
