FROZEN SEMEN SHIPMENTS

PROCESS SUMMARY:

The shipment process begins when Canine Cryobank Inc. receives two documents; a Frozen Semen Authorization form (filled out by the semen owner) which authorizes the release of stored semen, and a Frozen Semen Order Form (filled out by the bitch owner or new semen owner) that tells Canine Cryobank where to send the semen.

Canine Cryobank then prepares the semen for shipment, bills the party providing the Frozen Semen Order Form a handling charge, and delivers the semen to FedEx for shipment. FedEx separately bills the account provided by the recipient directly for the shipping charges.

The recipient uses the shipped semen, and then returns the empty shipping tank to Canine Cryobank. Again, FedEx bills the provided account number directly for the return shipping charges.

STEPS IN SHIPPING PROCESS:

The semen owner must fax the completed form to Canine Cryobank before any shipment processing may begin.

2. COLLECT REQUIRED INFORMATION:

A) FedEx account number for shipment. Call 1-800-GO-FEDEX or visit www.fedex.com to set up an account, if necessary.
B) Stud dog information, including registration number (if applicable), breed, call name, and formal name.
C) Semen owner information (person authorizing transfer or shipment of semen), including name, address, telephone number, and email address.
D) Bitch owner or new semen owner information, including name, address, telephone number, and email address.
E) Inseminating veterinarian or new storage facility information, including contact name, hospital/facility name, address, telephone number, fax number, and email address.

3. FILL IN THE REQUIRED FROZEN SEMEN ORDER FORM WITH INFORMATION COLLECTED ABOVE

Please note that all of the information listed above is required. Processing cannot begin until all information is provided.

Once the required information is entered into the form, a partially completed document will be created for you to print.

FOR INTERNATIONAL SHIPMENTS (OUTSIDE THE UNITED STATES OR CANADA)

Follow the above instructions (all of the same information is required), and contact Canine Cryobank, Inc. for further instructions. No action will be taken on any international shipments until all required paperwork, forms and clearances are delivered to Canine Cryobank. This may include import licenses, duty waivers, veterinarian clearances, etc. All import requirements are the responsibility of the receiving party or the semen owner (to be determined between them); Canine Cryobank will have no responsibility to obtain any required documentation, inspections, or permits. International shipments can be extremely complicated and time consuming, and Canine Cryobank Inc. has no responsibility beyond delivering the semen to the shipping company. All risk of loss for the semen is to be determined between the semen owner and the recipient.

CHARGES AND FEES:

Canine Cryobank charges the following handling charges for frozen semen shipments (Note these are shipment times, not delivery times):

 PREPARATION FOR SAME BUSINESS DAY SHIPMENT*: $375.00 + Shipping Fees
 PREPARATION FOR NEXT BUSINESS DAY SHIPMENT*: $250.00 + Shipping Fees
 PREPARATION FOR STANDARD SHIPMENT: $200.00 + Shipping Fees
 SPECIAL ARRANGEMENTS: TBD (Call for estimate)

*Please note that for same-day shipments, Canine Cryobank will make best efforts to ship the day the order is received, but we do not guarantee same-day shipment, Additionally, all information must be received by Canine Cryobank no later than 11:00 A.M. Pacific time to be processed that day.

*Also note that Sundays and Holidays as well as any day that FedEx does not accept shipments do not count as business days when computing shipping times and handling charges. For example, forms received Sunday for shipment on the following Monday are considered same-business-day orders. Likewise, if completed paperwork is received after 11:00 A.M. Pacific time, it is considered received the next business day for purposes of computing shipment times and handling charges.

Please be aware that these are SHIPMENT times, not delivery times. Shipments are sent FedEx Priority Overnight, and shipments within the US should arrive the next day that FedEx delivers packages. Delivery times for international destinations (including Canada) vary. If you require earlier delivery than that provided by FedEx, please contact us. Note that any special arrangements will be charged the same day shipment charge PLUS $75.00 per hour (one hour minimum) for Canine Cryobank staff time, and Canine Cryobank will not advance any funds to any shipping agent or company; payment arrangements for shipping charges are solely the responsibility of the recipient.
